Are You New To FFCS?
Begin your therapeutic process by completing the new client Intake documents. The information gathered is kept confidential in a HIPPA compliant database. Once we have received your intake, a FFCS administrator will be in touch to discuss benefits verification and/or scheduling an intake with your preferred clinician. Sessions will not be available for scheduling over the phone or via online booking until we have a fully completed intake packet. If you are using insurance as a funding source for sessions, your benefits will be verified prior to your intake session being scheduled. Please note that all information must be completed on your intake packet submission, including the financial guarantor form that asks for a credit card to be held on file. This information is mandatory for all clients to be kept on file for client costs such as copayment, deductible or no-show fees. Further details about policies and procedures are included in the intake packet.
